Clinical nurse specialists can be a major resource in the care and management of patients with chronic conditions because they are uniquely prepared to manage these patients, lead collaboration within and across health care settings and serve as the bridge between disciplines. This is the top finding of the NACNS Chronic Conditions Task Force.
